Core Responsibilities
- Conduct comprehensive initial assessments and regular reassessments of patients in their home environments
- Develop, implement, and continuously update individualized care plans in collaboration with the multidisciplinary care team
- Administer medications, wound care, infusion therapy, and other skilled nursing interventions as ordered
- Monitor and evaluate patient response to treatment plans and communicate changes to the supervising physician promptly
- Educate patients and family members on disease management, medication adherence, safety practices, and self-care techniques
- Accurately document all clinical activities using an electronic health record system in compliance with regulatory requirements
- Coordinate with discharge planners and case managers to ensure smooth transitions of care
- Participate in on-call rotations as required to support continuous patient care coverage
- Maintain compliance with all applicable state, federal, and accreditation standards governing home health practice
Must-Have Requirements
- Active and unrestricted Registered Nurse license in the state of Michigan
- Minimum of one year of clinical nursing experience, with home health or medical-surgical experience strongly preferred
- Current CPR and BLS certification
- Valid Michigan driver's license and reliable personal transportation
- Willingness and ability to travel to patient homes throughout the Sterling Heights and greater metro Detroit area
- Strong clinical assessment and critical thinking skills
- Proficiency with electronic health record documentation systems
- Excellent communication, time management, and organizational abilities
- Ability to work independently while maintaining strong collaboration with a care team
