Core Responsibilities

  • Establish measurable, functional goals in collaboration with patients and their families
  • Monitor and document patient progress accurately and in compliance with all regulatory requirements

  • Educate patients and caregivers on home exercise programs, safety strategies, and fall prevention techniques

Must-Have Requirements
  • Active Michigan Physical Therapist license in good standing
  • Graduate degree from an accredited physical therapy program
  • Minimum of one year of clinical experience, preferably in home health or a related setting
  • Valid driver's license and reliable personal transportation
  • Strong working knowledge of Medicare home health regulations and documentation standards
  • Demonstrated ability to work independently with excellent time management and organizational skills
  • Proficiency with electronic medical records systems
  • Outstanding interpersonal and communication skills with a patient-first mindset
