Core Responsibilities
- Administer prescribed medications and treatments accurately and in accordance with established policies and procedures
- Monitor and document resident vital signs, health changes, and responses to treatment on an ongoing basis
- Perform wound care, catheter care, and other skilled nursing procedures as directed by the care plan
- Communicate resident status updates clearly and promptly to the supervising registered nurse and attending physician
- Assist in developing, reviewing, and updating individualized resident care plans in collaboration with the interdisciplinary team
- Respond promptly and calmly to resident needs, emergencies, and changes in condition
- Provide emotional support and education to residents and their family members
- Supervise and delegate appropriate tasks to certified nursing assistants while ensuring quality care delivery
- Maintain accurate, timely, and thorough documentation in electronic health records
- Follow all infection control, safety, and regulatory compliance standards
Must-Have Requirements
- Current and valid Connecticut LPN license in good standing
- Graduation from an accredited Licensed Practical Nurse program
- Minimum of one year of clinical nursing experience, preferably in a long-term care, skilled nursing, or assisted living environment
- Demonstrated ability to work effectively within a multidisciplinary team setting
- Strong written and verbal communication skills
- Proficiency with electronic health record systems or willingness to learn quickly
- Ability to remain composed, organized, and compassionate under pressure
- Reliable attendance and a strong commitment to professional accountability
- CPR and Basic Life Support certification required or must be obtained prior to start date
